Medication Records (eMAR)

eMAR

Electronic medication administration records — what was given, when, and by whom, checked against the five rights.

Key features

  • Shows what is due, for which patient, at what time
  • Records the time each dose was actually given
  • Reminds staff when a dose becomes due
  • Checks the five rights — patient, drug, dose, route, time
  • Records the reason when a dose is held or omitted
  • Full administration history as a timeline

Overview

The eMAR makes medication administration safer by showing exactly what is due and when, and recording what was actually given as it happens.

Why it matters

Medication error is one of the most common sources of avoidable harm in healthcare. Automated checking at the bedside, plus a record that can be reviewed afterwards, reduces both the chance of a mistake and the difficulty of investigating one.

How it connects

Medication orders arrive from order entry, appear in the nurse station, and draw on pharmacy data for what was dispensed.
